A crucial aspect in outpatient care is the coordination of care requirements specific to each patient and the coordination among the care partners involved. APST offers a digitally enhanced managed care program for better coordination and networking among patients, doctors and providers (i.e., “care partners” such as suppliers of assistive technology devices and therapeutics, pharmacists, nutrition therapists, etc.).
We offer managed care to patients with particular care specifications and needs.

• complex chronic diseases (e.g. Multiple Sclerosis)
• rare diseases (e.g. Amyotrophic Lateral Sclerosis)
Patients with particular needs:
• high expenditure for administrative and organizational matters for patients and medical partners
• great need for specialization to ensure high quality care provision
• great need for coordination among the various doctors and care providers involved
Managed care facilitates communication and networking among patients, doctors and care partners. Managed care extends to the following care fields:
Based on the symptoms and complaints you present with your doctor will establish your individual need for assistive technology devices. This needs assessment will then be communicated to APST. APST will find suitable qualified providers of assistive technology devices with appropriate competencies. The objective pursued by APST is to coordinate patients and care partners.
Care needs assessment
A patient is diagnosed with a specific disease and a specific severity. Following clinical assessment, the treating doctor will suggest a treatment concept. Based on the patient’s physical impairment the doctor will indicate the need for an assistive device (e.g. a wheelchair) to be provided to the patient. In a personal conversation, the doctor will advise and inform the patient of the need for a device or therapy and of any associated administrative steps to be taken.
Care needs coordination
Once a patient has read, understood and signed the patient information and consent form, they will commission an APST coordinator to organize their various care requirements and to contact a specialized care partner on their behalf. Following consultation with a doctor, the coordinator will be informed of the patient’s individual care needs and subsequently get in touch with the patient to gather more information on their personal living situation. Based on the needs assessment performed by the doctor and the patient’s social situation ascertained in the personal interview suitable care partners will then be contacted. In our example, a medical equipment and supplies store specialized in the supply of wheelchairs is contacted. As the various activities of the care partners involved are coordinated, there is a guarantee that the assistive technology devices actually meet the patient’s requirements and can be utilized to maximum effect.
Complementing standard care
APST Managed Care with technological support via the APST internet portal is embedded in the established standards of care. Responsibility for medical necessity and rentability of therapies, assistive technology devices and other medicinal products remains with the attending doctor. APST’s contribution is to act on behalf of the patient and contact a suitable provider who has been contracted by the patient’s health insurance company and is adequately qualified and experienced to see to the patient’s particular needs.
APST supports patients in the Berlin and Brandenburg areas who are looking for a suitable physiotherapist and/or speech therapist. APST coordinates a network of therapy practices in the Greater Berlin Area.
A doctor establishes the indication for certain therapies, such as physiotherapy, and issues a prescription accordingly. In a subsequent step, the patient will commission an APST coordinator to identify a specialized therapy practice. Three key criteria apply in the process: the therapist(s) must be duly qualified to meet the patient’s special needs, the practice must be close to the patient’s home, it must offer the option of visiting and treating the patient at their home, if applicable, and have sufficient capacity to care for the patient. The coordinator will forward the care request and the doctor’s prescription to the therapy practice that will then get in touch with the patient to arrange an appointment.
We can identify three key areas in which patients benefit from coordinated physiotherapy and /or speech and/or occupational therapy provision by APST:
The care partners in the APST network are all specialized in the treatment of neurological patients. Fundamentally, each fully certified therapist is allowed to treat neuromuscular diseases. However, in addition to completing the formal training pathway, daily experience in the treatment of neurological conditions is vital to the therapeutic quality a therapist can deliver. Therapists may also gain special expertise in the treatment of neurological and neuromuscular diseases by attending further training courses and observing neurologists and other medical specialists in their work
APST coordinates the prescriptions issued by doctors and forwarded to patients and providers. The standardization of issuing initial and repeat prescriptions and the keeping of electronic records on the APST portal results in the streamlining of the administrative tasks of doctors, patients and therapists. As prescriptions are processed electronically in the first place, the number of phone calls and associated disruptions to the daily routines in a practice and/or outpatient setting are reduced substantially.
Feedback on the treatment success
The feedback tool offered on the APST portal is an innovative way of monitoring and improving the quality of care and services. Patient ratings for physiotherapy, speech therapy and occupational therapy enable therapists to evaluate the subjective benefit of the treatment they administer and detect any burdensome aspects. For patients, access to their personal electronic account is password secured. The electronic patient record captures any therapies currently being administered and offers the patient the option of rating the efficacy of each therapy he is receiving. Feedback is strictly confidential, a fact which distinguishes our platform from public physician and therapist rating and review sites.
To utilize the APST services, patients must fill in and sign the patient information and consent form for the APST scheme and associated data exploitation. The form is available from APST upon request
Drug therapy represents a therapeutic key element. The modern organizational structure and electronic documentation offered by APST simplifies patient care for patients, doctors and pharmacists alike, and improves drug safety and treatment compliance.
Electronic medication management plan
On the internet portal www.ambulanzpartner.de patients and doctors may get a printable overview of the current and past medication on the patient’s individual electronic medication plan. Patients may furthermore rate the medication and services they have received and thus contribute to improving care processes as such.
Improvement of drug safety
APST cooperates with specialist pharmacies that offer a service hotline to patients in the APST care network. Patients, relatives, care professionals and doctors may furthermore receive counselling on current drug therapies. This service is available from Monday to Friday from 9:00 hrs. - 17:00 hrs. CET.
Personal contacts are pharmacists specialized in the medication issued to patients in the APST care network. They can clarify any issues with regard to medication intake and thus reduce any risks and adverse drug reactions.
Simple and quick medication supply
The APST medication management option offers patients free delivery of any medication they have been prescribed to their home so they do not have to go to the pharmacy themselves. The prescription is processed on the day of issue by the cooperating pharmacy. As all the partner pharmacists are specialized in the APST patient group, specific medication is always in supply or can be delivered promptly. Once the medication has been prescribed, repeat prescription orders can be placed via the telephone at the patient’s convenience

Recording copayment exemption
Patients may use the services offered by APST free of charge. In line with German legislation, pharmacies are obliged to charge patients with public health insurance cover a supplementary amount for most medicines they receive and to forward the amount to the respective health insurance company. If you are not exempt from such copayment or have private insurance cover you will receive an invoice from the cooperating pharmacy.
The cooperating pharmacies in the APST care network offer the option of electronically recording the copayment patients have made and to inform them once they have reached the copayment threshold. Patients may then submit the respective document issued by the pharmacy to their public health insurance company for reimbursement.
