he ALS functional scale (Amyotrophic Lateral Sclerosis Functional Rating Scale revised (ALSFRSr)) is a questionnaire that enables people with ALS to record and evaluate the progression of their disease.
How the FRS functional scale works
The FRS questionnaire consists of 15 questions on physical functions (e.g., speaking, writing, walking). There are 5 options for each question to assess whether an impairment is present and how severe it is. A maximum total score of 60 can be achieved on the scale. The higher the total score, the lower the motor deficits.

You can access the SMA self-assessment questionnaire (FRS function scale) via the “Complete” button on the start page. The questionnaire should be completed every 4 months. The days until the next response are counted down and displayed.

The FRS functional scale consists of 15 questions on the functions of the head, arms, legs, trunk, and breathing. The five possible answers per question are rated on a point scale from 0 to 4. Each answer option is briefly explained.

Result with total value and progression value
After answering all 15 questions on the FRS functional scale, the total score is between 60 and 0 points. The total score of the FRS functional scale and the progression score is displayed after the questionnaire has been completed. The progress value can only be calculated if at least two fully completed questionnaires have been recorded.
