Are there physiotherapists who specialize in ALS?
Specialization in ALS is not established in the training of physiotherapists. Therefore, the majority of therapists cannot be assumed to have specialized training or professional experience in ALS. Effective physiotherapy (and occupational therapy and speech therapy) can also be provided by therapists who have no specialized knowledge of ALS. Knowledge of the treatment of motor disorders (such as stroke and muscle diseases) is essential, as the basic principles that apply to the treatment of other motor disorders also apply to the treatment of ALS patients. Against this background, specialization in ALS is not absolutely necessary.
However, it has been shown that a specialized experience of therapy centers for ALS has some advantages. Specialization includes targeted treatment of symptoms of the first and second motor neuron (especially spasticity therapy) and knowledge of palliative physiotherapy. Another competence of specialized therapists is the adaptation of therapy to the course of the disease. Initially, treatment is aimed at preserving motor functions, while later on the focus is on avoiding consequences (arthrosis, thrombosis, contractures, lymphoedema) and palliative symptom relief. The differentiation of therapy between motor and palliative goals can be realized in a special way if there is special experience.
The provision of treatment guidelines and physiotherapy research in ALS (with the provision of results via scientific publications) should enable more physiotherapy practices to offer a specialized range of therapies for people with ALS in the future.
