Patients, relatives, doctors, pharmacists, nurses, medical supply stores, therapists and other care partners are connected via Ambulanzpartner.de and all care processes are made recognizable. Our case management serves to facilitate care and avoid incorrect care.

History
The idea for APST was born in 2009 in the outpatient clinic of Prof. Dr. Thomas Meyer and Prof. Dr. Christoph Münch at the Charité in Berlin. The two neurologists work in the outpatient care of severe neurological diseases such as amyotrophic lateral sclerosis (ALS). A major barrier to success in their day-to-day outpatient work was the lack of coordination between patients, doctors, therapists, medical products and services. It was this lack of coordination that led to the creation of APST. This innovative service for coordinating outpatient care has been in existence since 2010. The Internet portal Ambulanzpartner.de was developed in 2010 in cooperation with doctors, therapists, scientists and IT specialists. Ambulanzpartner.de has been online since March 2011 and has been undergoing agile further development ever since.

The Motor Neuron Disease Network (MND-Net) is an association of 17 university centers nationwide for the treatment of amyotrophic lateral sclerosis (ALS). The MND-Net is a research network for basic, therapeutic and care research into ALS. APST supports the clinical researchers in the network with its Internet technology.
The Deutsche Gesellschaft für Muskelkranke e.V. (DGM) is a self-help organization for people with muscle diseases and neuromuscular diseases. It offers those affected and their relatives medical advice, social counseling and information on all aspects of muscle diseases.
The National Action Alliance for People with Rare Diseases (NAMSE) is a coordination and communication body with the aim of improving patient care for people with rare diseases. The alliance’s action plan includes ambulance partners in the information management field of action.
ALS-mobil is an association that informs, advises and supports people with amyotrophic lateral sclerosis (ALS) and their relatives in questions of care options, mobility and the reorganization of personal life concepts.

Promotion
The Federal Ministry of Education and Research (BMBF ) is funding ambulance partners in several research projects. The research projects deal with the possibilities of digitalization and internet-based networking of people, services, products and processes in the healthcare industry. Five projects have already been successfully completed (ROBINA, SmartHealthNet, JPND CarE-ALS, ProDigA, ArNe).
The European Social Fund for Germany (ESF) supported the research project ProDigA (digitally supporting care processes for the design of good interaction work) as part of the “Future of Work” program of the Federal Ministry of Education and Research.
The Georgsmarienhütte Steelworks Foundation aims to support projects and initiatives that make sustainable regional and supra-regional contributions to successful coexistence in society. With the project “Conception, development and operation of a web-supported care center for people with amyotrophic lateral sclerosis (ALS)”, the foundation supported the nationwide provision of the service architecture of the Internet portal Ambulanzpartner.de and the development of a specialized care network for ALS.
Investitionsbank Berlin (IBB) is the development bank of the State of Berlin. With its business development activities, it contributes to the development of Berlin as a business location. APST was one of the university spin-offs supported by the state of Berlin that prototyped a hybrid technology and service product and put it into productive operation. The development of the Ambulanzpartner Internet portal was funded by the Technologiestiftung Berlin (TSB) through the Investitionsbank Berlin.
