Experience has shown that the organizational steps of care are a burden or even an excessive demand for patients, relatives, but also for the doctors, practices and outpatient clinics providing care. This is where the care coordination of Ambulanzpartner comes in. The company was founded in April 2011 at Charité’s ALS outpatient clinic and is active throughout Germany.
